Because of the nature of our clinic and the volunteer physicians, we cannot accept walk-in patient for counseling or medical care. In order to register as a patient or counseling client a person must:
Complete a registration application at the clinic or have:
One faxed to them
Provide a photo ID (ex. drivers license; TX I.D.; birth certificate)
Provide proof of income (ex. 3 paystubs; government award letters; notarized letter)
Provide proof of current address (ex. utility bill)
Once a medical patient’s application is processed they will receive an order for laboratory services. After lab work is processed and sent to the clinic an appointment will be set with one of our providers. Once a counseling patient’s application is processed they will be called by the Counseling Services Coordinator for screening and appointment dates.
