For Patients

HOPE only sees patients on an appointment basis. New patients must complete an application for services. Necessary documents include a picture ID, proof of income and proof of address. HOPE is not an emergency clinic. If a person is in need of immediate care please call 911 or go to the Emergency Room. In addition, a Crisis Line is available for mental health and substance abuse services at 1-877-289-7199

A patient’s application is submitted and processed they will be called for an appointment with a medical provider. During a patient's visit they may be seen by one or more individuals on the health care team. This includes the Doctor or Physician's Assistant, Medical Assistant, Behavioral Health Specialist, Care Coordinator, or Transitional Specialist. Your health care team will work with you on your health goals.

Walk-in hours have changed and will be as follows:

Walk-ins for clinic and appointments will be held only on Tuesdays after 1pm start April 1, 2019.

Walk-in for labs will only be permitted on Monday's, Wednesdays, and Fridays from 7:30am-11am and Tuesdays and Thursdays from 8am-12pm.


During the visit you may be given an order for lab work or any necessary referrals. HOPE is not responsible for third party payments including lab work. As part of your health plan, you may be required to pay for lab work necessary to your health. 

A client must complete an application for counseling services as well as submit necessary documents. Once the application is received, a person will be called by one of the HOPE Therapists to further assess needs. Your case will be assigned to one of the therapists and you will be called for an appointment.
