As part of the care team, HOPE’s Care Managers work with patients to help develop individualized health plans with patients according to their own needs and health goals. During an integrated health care session, the care coordinator works as part of the health team to assist the physician, transitional specialist, medical assistant, and behavioral health coordinator assess for the team based medical care.
Care Coordinators and the Case Mangers also help patients with referrals to speciaists in the community. While HOPE is unable to provide payment for third party services (lab work, surgeries, etc), the Case Workers assist patients in finding avenues for care.
HOPE now provides the resource for patients to apply for Hidalgo County Indigent Program on Thursdays with an eligibility specialist stationed at HOPE. Leeroy Rosales, Eligibility Specialist, Health & Human Services Department comes in on Thursdays from 8am-5pm (scheduled appointments only) to determine if patients can qualify for any of the offering programs.
