Enrollment

Service Description

All participants at HOPE must complete enrollment forms and provide a copy of their picture ID, proof of income, and proof of address. Enrollment may take from 10-30 min. Once the enrollment process is complete, a patient will be given an appointment for lab work and the provider (if medical) or they will be called by one of the therapists for an appointment (counseling).
