HOPE provides primary medical care aimed at improving individuals' overall health, including but not limited to:
Laboratory Exams
Obesity
Medications
Hypertension
Screenings and
Diagnostic Testing
Diabetes
Our medical team also works with our behavioral health consultants and care coordinator to create individualized health plans with you that address total wellbeing.
We have a Vaccine Outreach Specialist to answer any COVID related questions you may have. Check our events tab for days when we offer COVID vaccines.
We primarily treat, prevent, and educate on:
We can make referrals to other clinics for services we do not provide,
such as minor surgeries, x-rays, and mammograms.
These outside services can also be low-cost.
