Service Description

HOPE provides primary medical care/integrated team based services to the uninsured aimed at improving an individual's overall health. HOPE's medical providers are volunteer medical doctors and physician assistants as well as residents from the University of Texas Rio Grande Valley. Because services are provided by volunteers, we encourage appointments. All patients must be established with HOPE prior to receiving services and the first step is to call the enrollment specialist at 956-994-3319 to complete your application and schedule a lab work appointment. HOPE's medical services are integrated - meaning when an individual seeks treatment they will have access to a care coordinator who will help them achieve their health goals and create a health plan which may include medical services, nutrition, behavioral health, case management, medication assistance, and/or counseling. We understand that at times there may be financial barriers to receiving care. However, HOPE's mission is to ensure everyone who needs medical attention receives it. Medical care is based off of donations - We ask for a $5, $10, or $15 donation, however, no patient is every turned away for a inability to donate. Patients are responsible for the cost of their lab work that is conducted on or off site. At times, a physician at HOPE may require additional testing for the patient such as X-Rays, MRI, etc. If this is the case, the patient meets with the Case Manager to discuss the procedures and payment options.
